Improving detection of Breast Cancer with compression at optimal pressure

Worldwide, over 150 million mammograms are performed every year. Recent studies demonstrate that a too low pressure leads to a higher recall rate and false positives. Too high pressure reduces detectability of breast cancer. The only way to achieve pressure guidance in a consequent way during mammographic positioning is by using the Sensitive Sigma™ Paddle.

Optimize your compression

The relationship between pressure applied during compression of the breast and screening performance was investigated in the study “Influence of breast compression pressure on the performance of population-based mammography screening” which was published in Breast Cancer Research 2017, 19:126 (DOI:10.1186/s13058-017-0917-3). Researchers computed the compression pressure applied to a series of 113,464 screening mammograms. Screening performance measures were determined for each group. Cancer detection rate and the positive predictive value were optimal in the middle compression category.

The new innovative Sensitive Sigma™ Paddle is designed to seamlessly integrate with existing and new mammography systems. Therefore, the design looks much like the existing paddles. The paddle interface differs per mammography unit and must meet strict standards and regulations. The Sensitive Sigma™ Paddle is open to any mammography system. Current breast compression guidelines and protocols are based on force. Indicated on the mammography unit in Newtons (N or daN). This results into a large variety of interpretations and a wide range of applied forces, where smaller breasts are relatively more compressed.

The Sensitive Sigma™ Paddle solves the current problems of under- and over-compression, by standardization around 75mmHg pressure. Under-compression results in images with too low contrast, motion artefacts and a higher average glandular dose (AGD). Over-compression results in unnecessary discomfort, vanishing tumors and negative patient or client perception. Clinical trials show positive results and improved experience1 with the use of Sensitive Sigma™ Paddle.

75 mmHg as clear guideline

The procedure of breast positioning is exactly the same as usual, but when our paddle touches the breast, innovative sensors collect information about the breast which was not available before. This allows us to personalise breast compression and to take breast size and tissue stiffness into account. LED light indicators on the paddle light up while the pressure on the breast increases until the optimal pressure is reached. The radiographer is always in control of the compression, the Sensitive Sigma™ Paddle provides real-time visual feedback. If preferred, the last part of the compression can be done manually.
